Location: Ballston Spa, NY
Pay range - $33-39k/year, plus additional incentives

We are seeking a Case Manager in a Human Services program for an immediate opening with our client. This is a full time role, and it is expected to include some on-call coverage dealing with at-risk youth. You’ll be involved in working with other community organizations, training of staff members, assisting with volunteers, and documenting related activities.  This position is critical to the program as it provides these youth with needed residential, and support services. Your role as Case Manager will be to develop and monitor these relationships, as well as to advocate and connect these youth with supporting activities. You’ll be provided with ample training and additional support.


  • Work with local support services to assist at-risk youth with individual service needs.
  • Perform Intake Interviews for new residents, coordinating with various community service agencies for additional information.
  • Develop individual service plans for each client including short and long term goal planning based on client needs
  • Perform routine assessments to evaluate client progress, updating related documentation regularly
  • Review client activity with staff members to review successes and challenges daily.
  • Work with clients on core goals, updating activities as appropriate
  • Comply with all internal regulations as well as state/regulatory requirements; maintaining accurate and up to date files at all times.
  • Update billing and financial information regularly for finance department review
  • Communicate with clients at regular intervals during residency and afterwards to follow up on action items and client status.
  • Participate in and organize regular staff meetings related to client care. 


  • Bachelor’s degree required
  • Experience developing trusting relationships with clients, and supporting community service agencies.
  • 2 years or more experience working in Human Services
  • Case Management experience preferred
  • Clean and Valid driver’s license
  • Computer skills to type reports, document incidents, use email for correspondence
  • Ability to communicate clearly in person, on the phone, and via email
  • Ability to take on-call shifts as needed
